If you observe any of the following clinical indicators in your patient or loved one, it may be time to consider a hospice evaluation:

• Multiple hospitalizations or emergency room visits
• Recurrent or persistent infections
• Decline in the ability to perform daily activities
• Progression of chronic or co‑existing conditions
• Unintentional weight loss or weight gain
• Frequent falls or noticeable changes in mobility
• Changes in oxygen saturation or increased shortness of breath
